Application for Admission

A complete Application to Williamsburg Skin Institute includes each of the following required materials:

1. 1. Application for Admission
   2. Copy of valid ID
   3. Professional passport compliant 2" x 2" color photo. It must meet the following requirements:
         - Sized so that the head is approximately 1 inch from the bottom of the chin to the top of the head;
           - Taken in front of a plain white background;
           - Be a full-face view, directly facing the camera with a neutral facial expression.
   4. Application Fee $75.00
